Additional Information:
Since last fix (which was done via another deal dispute like this) only lasted about a month, then I had to chase for fix another 2-3 weeks, after each fix, the upgrade only lasts few days, then again I have to chase him for 2-3 weeks to have it only last another few days.
This time again, I contacted him after only 2 days of his last fix lasting, and getting ignored. Opened ticket several time, he is closing these ticket's without any replies, DMed him directly and still only says will be fixed but then proceeds to close ticket without any explanation. 

I no longer want this service, it seems like I am doing more work than him as I have to chase him for weeks only for the service to last me a day or two. Since it's a life time warranty, I want a refund if possible as this service is not it.

Which I have followed multiple times, and as you can in the last screenshot you have admitted that it was your bad (Screenshot), and in another screenshot of the ticket it clearly shows me sending all proofs etc yet it was still closed (Showing ticket with proofs).
And also in PM you have said multiple times you will fix it then ignored for another few days, in many cases weeks before any replies from you. Other disputes also show the same thing.
