Other languages would be a mess, you need someone able to speak those languages in order to moderate the chat and shoutbox moderators aren't a thing.
Spam chat is useless, Premium chat would be dead or it would kill the main chat, it requires extra moderation and staff would have to talk in both chats simultaneously.
