Supreme Thread Vouch Bug

Tried leaving a vouch on a thread and any Thread that I click on to leave a vouch the screen loads up white and there's nothing to click I assume its just some minor bug with either theme + plugin causing this or probably low memory w the hosting company or maybe something else
I tried multiple devices + browsers with cookies cleared and everything still returns an Error so not sure what's the issue visiting other pages is not a problem though or browsing other threads its only when I try to leave a vouch

Here is an example this is @KSZ Intensehost thread for rdps when I try to leave a vouch on the thread it shows this !
